A CASE OF ACUTE TUBULAR NECROSIS TREATED WITH HEMODIALYSIS


Abstract
A case of acute tubular necrosis due to mercury ingestion who was treated successfully with Kolff¢¥s twin coil artificial kidney is presented. Te patient was 26 year old Korean male who became oliguric after ingestion of approximately 5gm. of bichloride of mercury. The patient became gradually uremic from the 4th day after ingestion of toxic agent, inspite of rigid conservative management for acute oliguric renal failure. On the 6th day, there were evidences of hyperkalemia on E.C.G. and hemodialysis was perfromed for six hours on the following day using Kolff¢¥s twin coil artificial kidney. There was prompt disappearance of uremic symptoms after initiation of hemodialysis and B.U.N. was reduced from 150mg.% to 33.3% and creatinine 10.3 to 6.9mg.% 82.54gm. of urea and 4.5gm. of creatinine were removed from the body at the same period, and dialysance of urea was 73.3ml/min. and creatine 53.2ml./min. Diuretic phase followed immediately after hemodialysis, and on the 18th day N.P.N. was 126mg.% and creatinine 6.37mg.% The patient was discharged asymptomatic on the 20th day. A brief review of conservative management of acute renal failure, and history. principles and clinical application of artifical kidney was presented.
